SOCIAL SERVICES

The Medical Social Worker provides assistance to home-bound patients who have social and emotional issues related to their medical needs. These problems may include, but are not limited to:

  • Home safety
  • Community resources

The social worker can also assist with nursing home placement in the event that becomes necessary.
Specific services and duties include, but are not limited to:

  • Accessing community and financial resources
  • Assessment of the patient’s living environment and financial status in relation to the disease process
  • Assisting the members of the health care team and the patient’s family to understand the significant social and emotional factors related to the patient’s health problems
  • Locating alternate living arrangements
